# Sample Submission Form (COC)

## The sample submission form is generated once you submit samples through GXLab to any of our partner labs. Download and print this COC and package with your physical soil samples to be sent to your preferred lab.

The sample submission form contains important information required by the laboratory to conduct analysis. This document includes billing information, biosecurity details and of course your specific testing requirements.
